Many countries claim that this popular recipe originated in their culture, however there is no definite evidence of where cabbage rolls first emerged.
However, most stuffed cabbage rolls in the United States start with a filling made of ground meat, rice and chopped vegetables. I remember her becoming frustrated when trying to roll the filling into the cabbage. The cabbage was either too stiff for it to roll easily, or the leaves would be too tender and it would tear and the filling would fall out. This memory was enough for me to stick to cabbage roll casserole rather than making true, old fashioned, stuffed cabbage rolls.
But that all changed when I found a few tricks to making the rolling of the cabbage leaves easier. Rather than pulling off individual leaves, we prefer to place the entire head in the water.
It makes it much easier to pull off individual leaves from the core and prevent tearing. We then remove the thick vein at the bottom of each leaf. This results in a v shaped cut which comes in handy when it comes time to roll the filling into the leaves. We place the filling right above the v-cut. Then fold the cut ends of the cabbage on top of the filling and roll. The remainder of the unused cabbage is then sliced and placed in the bottom of your Dutch oven or casserole pan. You may need to an extra leaf or two during the rolling process.
The prepared stuffed cabbage rolls are placed seam side down in the pan and topped with a tomato sauce. Once baked, we allow the cabbage rolls to sit and rest before serving.2019-03-05

But while leaders once worried about the shift of surgeries from high-revenue inpatient settings to lower revenue hospital outpatient HOPD settings, the rapid growth of ambulatory surgical centers ASCs is stoking even greater fears about the ability to maintain hospital surgical revenues.

Most of the previous shift from inpatient to outpatient settings was driven by advances in technology and technique which made it possible to perform previously intensive surgeries in a less-invasive manner, shortening recovery time and reducing infrastructure requirements. Yet, today's outmigration is fundamentally different because it's principally driven by purchasers.
Here's how payers, consumers, and physicians have contributed to the latest shift. CMS has been particularly aggressive recently, such as by pushing site-neutral payments to compress HOPD rates while also modifying payment rate increases to make ASCs more attractive. They've also expanded the list of procedures eligible to be added to the ASC covered list, most notably adding 12 cardiac catheterization procedures, and suggesting adding total and partial hip replacements as well.
Private payers have followed CMS' lead by adjusting contracts to lower HOPD reimbursement and requiring hospitals to perform a certain percentage of outpatient cases in ASCs in order to receive reimbursement increases over time.
Finally, some payers have directly altered patient benefit design to incentivize ASC choice. Your ASC services outlook primer. UnitedHealth Group separately owns UnitedHealthcare. There are many factors encouraging physicians to shift surgical cases to ASCs. Many ASCs have—and continue to—advertise appealing equity options to physicians. But even for physicians without a financial stake in ASCs, the centers can be appealing. Payers often offer higher physician payment rates in ASCs, and some provide cash bonuses on a per case basis for every appropriate patient that receives care in an ASC as opposed to a HOPD.
Finally, ASCs tend to have higher throughput and be more efficient, which can allow physicians the ability to perform a higher volume of surgeries. As more high-revenue procedures get approved for ASCs, operating in ASCs will likely become even more attractive for physicians. Hospitals and health system leaders have three strategic options for how to proceed in the growing but competitive ASC market. First, they can attempt to reverse the outpatient shift by engaging in protectionist tactics intended to keep cases in the inpatient setting and convert ASCs to hospital-based reimbursement rates.
In the long term, however, this approach is likely ineffective with consumers and payers. Second, health systems could overlook the outpatient setting altogether by prioritizing the inpatient business and not actively undermining outmigration or investing in ASCs. Given the relatively lower growth of inpatient care and higher growth of outpatient procedures, organizations will struggle to meet revenue goals without capturing outpatient growth.
Third, leaders can implement a proactive strategy to compete for outmigrating cases. As part of last week's announcement, Fiat Chrysler CEO Mike Manley said the investments will allow the carmaker to pursue new opportunities, "including at least four plug-in hybrid vehicles and the flexibility to produce fully battery-electric vehicles.
That part of the announcement wasn't entirely unexpected.

While the late Marchionne, Manley's predecessor, was clearly a battery-car skeptic, he had come to accept that Fiat Chrysler would have to follow the lead of its competitors — as well as the push from regulators for more energy efficient cars, especially in key markets like the U.
During the company's investor day event last June, Marchionne revealed plans for a number of new battery-based models. These include high-end "halo" vehicles such as the planned Alfa Romeo 8C, a plug-in hybrid supercar. But the new announcement "gives more clarity to what they've previously said," according to Sam Abuelsamid, a senior analyst with research firm Navigant.
For one thing, Manley is making it clear that Fiat Chrysler will design future products to use a broad range of powertrains, from gas and diesel engines to mild, "conventional" and plug-in hybrids, as well as pure battery-electric drivetrains. Marchionne's attention last year seemed focused on China, now the world's largest market for plug-based vehicles.
Driving that surge is the so-called new energy vehicle rule enacted in late It provides significant incentives to manufacturers and consumers to build and buy plug models. But last week's news suggests that Fiat Chrysler is now starting to see more opportunities in the U. If nothing else, said Abuelsamid, it has recognized that the competition will be driving the push to electrify as much as regulators.
Fordfor example, revealed plans in January to build an all-electric version of its F, the best-selling competitor to Fiat Chrysler's Ram A number of mainstream competitors are also working up plug-in hybrid and all-electric SUVs. At the high end, meanwhile, BMW has developed a new "architecture," or platform, that will underpin virtually all future models, including sedans, coupes and SUVs that can accommodate all possible types of engines.
The challenge will be to come up with battery drive systems that can appeal to Jeep and Ram buyers, some of the market's most traditional customers. They are offered with optional "mild" hybrid drivetrains using small battery-electric assist technology to not only improve fuel economy but also to boost performance and enhance on- and off-road driving capabilities. With future models, "They could come to market with products that are differentiated from what other companies are offering by enhancing the capabilities" of Jeep and Ram models.
Perhaps the bigger challenge, however, will be finding a way to turn a profit on future electrified models. While Tesla managed to claw into the black during the last half ofCEO Elon Musk warned of a probable loss during the first quarter and automakers have traditionally gone into the red — often, deeply into the red — with their battery-based models.
The equation has been improving, Manley said, as battery costs tumble. Even so, he cautioned that Fiat Chrysler expects it can recover only 60 percent of the incremental cost for electrified powertrain technology.
Buyers simply aren't willing to pay a premium. That's why the automaker is expected to continue playing things cautiously as it decides just how fast to roll out the new electrified powertrain options it is developing.
